I agree to take full responsibility for the football equipment provided by Peebles Football to my child and will return all items at the end of the season at the designated turn-in date in good condition, assuming normal wear and tear. All equipment supplied for use will remain the property of Peebles Football: Football helmet, football shoulder pads, blue game pants and game jersey. I agree that these equipment items will only be used in a Peebles Football practice and/or game and will not be used at home for recreational or any other purpose. Failure to return the items listed above by the equipment turn-in deadline at the end of the current season may result in my payment to the Peebles Football program in the amount of replacement for like items.
I agree to take full responsibility for the well being and liability of my child’s safety and health while participating in tackle football. I understand and agree that injuries and accidents can occur during any sporting activity and will not hold Peebles Football, it’s coaches, trainers, volunteers, property landlord, or other players liable for any harm or injury that may occur with my child while participating in Peebles Football. I agree to maintain health insurance for my child and take full financial responsibility for my child if uninsured without holding Peebles Football liable for any costs or damages. I am aware that the coaches are teaching proper head’s up tackling fundamentals for the safety of my child. I agree that if my child requires medications such as inhalers for asthma or any other medical condition, I agree to bring them to each game and practice for my child and/or provide them to the training staff if one is available. I also understand that it is my responsibility to insure that water or sports drink be brought to each practice and game for hydration for the health of my child.
